Request Copies of Your Medical Records

To request copies of your medical records, please download, complete, and sign the Authorization for Release of Information form for your Willis Knighton Health hospital facility. Completed forms, along with a copy of a valid ID (driver’s license, military ID, or passport), and any other necessary documentation may be mailed to the facility's health information management department.
