However, please note that some hospital-based physicians may not have a contract with all the above listed companies. Radiologists, pathologists, anesthesiologists, emergency room physicians, hospitalists and neonatologists submit their charges for professional services provided to you and are in addition to your hospital bill. To get information about network participation of any hospital-based physician, patients may refer to the online provider directory of their insurance company for the most up-to-date information, call the hospital admitting department, or call the physician’s customer service number. Patients may request specific information from their health insurance company as to what benefits are payable for hospital-based physicians.
Willis Knighton Health will send you a bill for services provided by the hospital. Physicians who care for you will bill their services separately.
In some cases, you may receive bills from doctors who consulted with the physician caring for you, interpreted test results or provided a specialized service. These medical providers may include pathologists, radiologists, anesthesiologists, CRNAs, emergency room physicians, cardiologists, surgeons and other specialists.

HEALTH CARE SERVICES MAY BE PROVIDED TO YOU AT A NETWORK HEALTH CARE FACILITY BY FACILITY-BASED PHYSICIANS WHO ARE NOT IN YOUR HEALTH PLAN. YOU MAY BE RESPONSIBLE FOR PAYMENT OF ALL OR PART OF THE FEES FOR THOSE OUT-OF-NETWORK SERVICES, IN ADDITION TO APPLICABLE AMOUNTS DUE FOR CO-PAYMENTS, COINSURANCE, DEDUCTIBLES, AND NON-COVERED SERVICES. SPECIFIC INFORMATION ABOUT IN-NETWORK AND OUT-OF-NETWORK FACILITY BASED PHYSICIANS CAN BE FOUND AT THE WEBSITE ADDRESS OF YOUR HEALTH PLAN OR BY CALLING THE CUTOMER SERVICE TELEPHONE NUMBER OF YOUR HEALTH PLAN.
