Information for Patients

Should you need to cancel or reschedule your appointment, please notify your physician's office as soon as possible.

A checklist of what to bring to your appointment:

• Insurance Information
  Please bring your insurance card or necessary insurance information.
• Medications
  Please bring the medications you currently are taking or have taken recently, whether over-the-counter or prescription.
• List of Personal Health Questions or Concerns
  Sometimes we forget all the things we wanted to tell or ask the doctor, so make a list of your concerns or questions before your appointment. And be sure to bring it with you.

Our patients' health, wellness and safety are of the highest importance therefore smoking is not permitted in any Willis Knighton Healthcare facility.
